Clonazepam
Class: Benzodiazepine (antiepileptic and anxiolytic)
Indications
- Adjunctive therapy for epilepsy (absence, myoclonic, and Lennox-Gastaut syndrome)
- Panic disorder and anxiety disorders
- Acute seizure clusters
Mechanism of Action
- Enhances effect of GABA at GABA-A receptors
- Increases neuronal inhibition, producing anticonvulsant and anxiolytic effects
Dosage
- Adults: 0.5 mg orally 2–3 times daily; may titrate slowly up to 20 mg/day in divided doses
- Children: 0.01–0.03 mg/kg/day orally in divided doses
- Adjust dose based on response, tolerance, and clinical condition
Major Side Effects
- Drowsiness, sedation
- Ataxia, dizziness
- Behavioral changes (irritability, aggression, paradoxical reactions)
- Memory impairment (rare)
- Dependence and withdrawal symptoms with abrupt discontinuation
Contraindications
- Hypersensitivity to clonazepam or other benzodiazepines
- Severe respiratory insufficiency
- Severe hepatic impairment
- Sleep apnea
- Myasthenia gravis
Compatible Solvent / Administration
- Oral tablets, orally disintegrating tablets
- Take with or without food
Special Precautions
- Monitor for sedation, cognitive impairment, and paradoxical reactions
- Taper gradually to avoid withdrawal seizures and rebound anxiety
- Use caution with other CNS depressants or alcohol
- Pregnancy Category D – avoid unless benefits outweigh risks
