Drug Class
Leukotriene Receptor Antagonist Anti-asthmatic Anti-inflammatory Mechanism of Action
Zafirlukast competitively blocks cysteinyl leukotriene (CysLT1) receptors in the airways, preventing leukotriene-mediated bronchoconstriction, airway edema, and inflammation.
Indications
- Prophylaxis and chronic treatment of bronchial asthma
- Prevention of exercise-induced bronchoconstriction
- Allergic airway disease
Dosage (General)
- Adults & ≥12 years: 20 mg twice daily
- Children 5–11 years: 10 mg twice daily
Administer 1 hour before or 2 hours after meals.
Side Effects
- Headache
- Nausea
- Diarrhea
- Abdominal pain
- Elevated liver enzymes (rare)
Contraindications & Precautions
- Hypersensitivity to zafirlukast
- Hepatic impairment – use with caution
- Not indicated for acute asthma attacks
Drug Interactions
- Warfarin (↑ INR)
- Theophylline
- Aspirin (may increase levels)
Pregnancy & Lactation
Pregnancy Category B. Use only if potential benefit justifies potential risk.
Key Notes
- Less commonly used than montelukast
- Requires twice-daily dosing
- Liver function monitoring recommended for long-term use
